The Accident Investigation Board Norway (AIBN), the Defence Accident Investigation Board Norway (DAIBN) together with the Marine Safety Investigation Unit of Malta and the Spanish Standing Commission for Maritime Accident and Incident Investigations (CIAIM) have conducted a joint investigation for the collision of the frigate HNoMS Helge Ingstad and the oil tanker Sola TS.

The incident

In the early hours of Thursday, November 8 2018, the frigate HNoMS Helge Ingstad, with 137 conscripts and permanent crew on board, and the tanker Sola TS, with 24 people on board and operated by the Greek shipping company Tsakos Columbia Shipmanagement (TCM) S.A. collided in the Hjeltefjord.

 

A total of seven watch standing personnel, including two trainees, were present on the bridge

 Under SOLAS regulation I/21 and MARPOL articles 8 and 12, each Administration undertakes to conduct an investigation into any casualty occurring to ships under its flag subject to those conventions and to supply the Organization with pertinent information concerning the findings of such investigations. Article 23 of the Load Lines Convention also requires the investigation of casualties.

 
Under the United Nations Convention on the Law of the Sea (UNCLOS), article 94 on Duties of the flag State, paragraph 7, “Each State shall cause an inquiry to be held by or before a suitably qualified person or persons into every marine casualty or incident of navigation on the high seas involving a ship flying its flag and causing loss of life or serious injury to nationals

El buque italiano de transporte de Gas, SYN ZANIA, ha sufrido una explosión, seguida de un gran incendio, cuando se encontraba en el puerto de Uttar Predesh.

El grave accidente se ha saldado con un fallecido, y 15 tripulantes evacuados que han requerido atención médica.

A bordo del SYN ZANIA viajaban 15 profesionales italianos y uno rumano.

La explosión, cuyas causas aún se desconocen, se produjo cuando el buque italiano se encontraba realizando operaciones de carga en la refinería de Petkim, Aliaga, Turquía, sobre las 23- 30 hora local, el día 1 de julio de 2019.

El incendio pudo ser extinguido sobre las 02-00 hora local del día siguiente.
